Authorizations and Waivers

Authorization Submission Form
To be filled out by the Authorizing Marshal(s) as well as the prospective combatant or marshal. This form must then be submitted to the Marshal Secretary within two weeks to be processed.

Temporary Authorization Form
To be filled out by the Authorizing Marshal(s) as well as the prospective combatant or marshal. This form must be done in conjunction with the Authorization Submission Form and is good for only 30 days from date completed.
